If your using this form to nominate a MJS Artist for Artist Of The Month (AoTM), please make sure you put something like "MJS AoTM Nomination" with the month and artist name.
If you are using this form to report a user, please make sure the user actually violates one of the MJS Rules, and the issue isnt a personal argument. One of the MJS Staff members may or may not contact you, but that doesnt mean the issue wasn't reviewed and/or taken care of.
